## Small movements can ease stiffness

For a knee or hip, take a short, easy walk. For a shoulder, slowly raise and lower your arm.

You aren't testing how much the joint can take. Stop before a mild ache turns sharp.

Break large jobs into smaller parts. Your joint gets a rest before it flares.

## Heat or cold may settle a mild flare

Heat may loosen stiffness before you move. Cold may feel good after activity if the joint is swollen.

Choose the one that feels better. Don't place strong heat or cold on bare skin.

Home care may make daily tasks easier. It shouldn't hide soreness that keeps getting worse.

## A clinic visit can add non-surgical choices

2. A cross-sectional study contacted 273 of 317 US centres offering direct-to-consumer stem-cell therapy, posing as a 57-year-old man with knee osteoarthritis. The mean advertised price of a unilateral same-day stem-cell knee injection was $5,156 (SD $2,446; 95% CI $4,550-5,762, n=65) and the mean claimed clinical efficacy was 82% (SD 9.6%; n=36) - a figure the authors describe as a gap between marketing claims and the published literature.

An analysis of internet-based marketing claims found widespread direct-to-consumer promotion of unapproved stem cell interventions by businesses based in the UNITED STATES - previously treated as a phenomenon of countries with lax medical regulation - and concluded that regulatory agencies must better oversee this marketplace.

In January 2025 federal district court orders resolving a joint FTC and Georgia Attorney General complaint permanently BANNED the co-founders of the Stem Cell Institute of America and several related companies from marketing stem cell therapy, and required payment of $5,155,146 in civil penalties and consumer refunds. In March 2024 the court had granted summary judgment on all counts, finding the defendants created and published false and misleading advertisements about the efficacy of stem cell injections. The affiliated clinic charged up to $5,000 per injection, almost exclusively to elderly and disabled consumers.

In a within-patient placebo-controlled trial, 25 people with BILATERAL knee osteoarthritis received bone marrow aspirate concentrate in one knee and saline in the other, acting as their own controls. Pain scores fell significantly from baseline in BOTH knees at 1 week, 3 months and 6 months, and the relief - described by the authors as dramatic - did not differ significantly between the BMAC knee and the saline knee.

6. RESTORE, the largest and most rigorously blinded placebo-controlled PRP trial in knee OA (n=288, participant-, injector- and assessor-blinded), gave three weekly injections of a commercial leukocyte-poor PRP or saline. At 12 months the change in knee pain was -2.1 vs -1.8 points (difference -0.4; 95% CI -0.9 to 0.2; P=.17) and the change in medial tibial cartilage volume was -1.4% vs -1.2% (difference -0.2%; P=.81). Twenty-nine of 31 prespecified secondary outcomes showed no between-group difference. The authors concluded the findings do not support the use of PRP for knee OA.

7. A meta-analysis of 34 RCTs (1,403 PRP knees vs 1,426 controls) found WOMAC favoured PRP over placebo at 12 months and over hyaluronic acid at 6 and 12 months, and favoured PRP over steroids on VAS pain, KOOS pain, daily function and quality of life at 6 months. Crucially, the authors state that the superiority of PRP DID NOT REACH the minimal clinically important difference for any outcome and the quality of evidence was low.

A meta-analysis of 27 Level I studies (1,042 PRP, 226 BMAC, 1,128 HA patients) found significantly better post-injection WOMAC, VAS and subjective IKDC scores for BOTH PRP and BMAC compared with hyaluronic acid - and NO significant difference between PRP and BMAC on any outcome score. This is the clearest published statement that the two most-marketed orthobiologics perform the same as each other in the knee.
